Patient Agreements
Communications consent and media release.
Effective August 18, 2026.
These agreements apply alongside our Terms of Service, Privacy Policy, and HIPAA Notice of Privacy Practices.
Communications Consent and Marketing Authorization
1. Consent to Receive Communications
I consent to receive communications from Soothe IV Med Spa, including but not limited to:
- Exclusive offers and promotions
- New treatment announcements
- Wellness tips and educational content
- Event invitations
- IV therapy educational series
- Membership program information
- Special discounts and welcome offers for new subscribers
2. Communication Methods
I understand and agree that these communications may be delivered through:
- Email newsletters
- Text messages (SMS)
- Phone calls
Consent to receive marketing communications is not a condition of purchasing any goods or services.
3. Understanding of Terms
- I acknowledge that standard message and data rates may apply for SMS messages
- I understand that message frequency varies
- I understand I can opt out of communications at any time by:
- Replying “STOP” or “OUT” to text messages
- Replying “HELP” to a text message for assistance
- Clicking the “unsubscribe” link in emails

4. Privacy Protection
- I understand that my contact information will be handled securely and confidentially
- I acknowledge that Soothe IV Med Spa will not share or sell my contact information to third parties
- All communications will comply with applicable privacy laws and regulations
5. Promotional Offers
- I understand I may receive a welcome offer for joining the mailing list
- I may receive exclusive subscriber-only promotions and early access to special offers
- Promotional offers may have expiration dates and terms that will be clearly communicated
By submitting my contact information and signing up, I confirm that I have read, understood, and agree to these terms regarding communications from Soothe IV Med Spa.
Media Release & Promotional Use
From time to time, Soothe IV Med Spa may capture photographs, video, audio, or other media of treatments, services, and the people who receive them. By receiving services from Soothe IV Med Spa, you grant Soothe IV Med Spa and its affiliates a perpetual, royalty-free, worldwide license to use, reproduce, edit, publish, and display such media on an anonymous basis — without including your name or other directly identifying information — for promotional, marketing, educational, and commercial purposes, including on our website, social media, advertising, and printed materials.
How to opt out. If you do not wish to have media of your treatment or of you used for these purposes, you must notify us in writing before your appointment by emailing care@sootheiv.com. Upon receiving your written opt-out, we will not use media of you for promotional purposes. This opt-out applies going forward and does not obligate Soothe IV Med Spa to remove or recall media already published prior to your request.
