Information We Collect
- Contact information you provide (name, phone, email, address).
- Health information you provide through intake forms, telehealth evaluations, and treatment records (handled under our HIPAA Notice of Privacy Practices).
- Payment information, processed through our payment provider — we do not store full card numbers on our own systems.
- Website usage information (pages visited, general location, device/browser type) collected automatically.
How We Use Information
- To schedule and provide services, communicate about appointments, and process payment.
- To provide prescription products following a valid provider evaluation.
- For marketing communications, only where you’ve opted in — you can opt out at any time by emailing care@sootheiv.com or using the unsubscribe link in any marketing message.
- To improve our website and services.
How We Protect Information
We restrict access to personal and health information to staff who need it to do their jobs, and we require vendors who handle protected health information on our behalf — including our electronic health records platform, scheduling system, and pharmacy partners — to sign Business Associate Agreements committing them to HIPAA-level protections.
Sharing of Information
We do not sell your personal information. We share information only with service providers who help us operate (payment processing, scheduling, pharmacy fulfillment), as required by law, or with your consent.
Your Choices
You may request access to, correction of, or deletion of your information (subject to legal recordkeeping requirements for health records — see our HIPAA Notice for health-record-specific rights) by contacting care@sootheiv.com.
