Peptide Membership Terms & Conditions

Membership Agreement — Soothe IV Med Spa Peptide Program. This agreement is between you (“you” or “Member”) and Soothe IV Med Spa (“we,” “us,” or the “Practice”). By enrolling and authorizing the recurring charge at checkout, you agree to these terms.

1. What your membership is

Your membership is a medical-supervision service — physician oversight, protocol management, check-ins, and care coordination. It is separate from the cost of any prescribed therapy or medication, which is billed individually at your member pricing and only after physician evaluation and current bloodwork. Your membership fee is not a payment for medication.

2. Recurring billing & automatic renewal

Your membership is billed at the monthly rate shown for your selected tier at checkout, charged automatically to your payment method on your monthly billing date until cancelled as described below. By enrolling you provide affirmative consent to this recurring charge. We will send an acknowledgment of your enrollment and the auto-renewal terms.

3. 3-month minimum commitment

Your membership begins with a 3-month minimum (three monthly payments). This reflects the clinical runway needed to titrate your therapy and evaluate results. The minimum applies to the membership service, not to any medication.

4. Early termination during the minimum

If you cancel before completing the three monthly payments, an early-termination fee equal to the remaining minimum payment(s) becomes due at cancellation (for example, cancelling after one payment leaves two payments remaining). This is a reasonable estimate of the onboarding and care costs incurred, not a penalty for medication.

5. Month-to-month after the minimum

After your third payment, your membership continues month-to-month. You may cancel anytime, effective at the end of your current billing cycle, by submitting a cancellation request before your next billing date. You will not be charged for cycles after the one in which you cancel.

6. How to cancel (easy cancellation)

Cancel by emailing care@sootheiv.com or replying to any membership email, or by calling (949) 229-2574 during business hours. We will confirm your cancellation in writing. No retention call is required to cancel.

7. Refunds

Membership fees for a billing cycle already begun are non-refundable because the supervision service for that period has been made available. The early-termination fee in Section 4 is likewise non-refundable. Amounts paid for medication are governed by the pharmacy’s policies.

8. Medical terms

Membership is not a prescription and does not guarantee that any therapy will be prescribed; not all applicants qualify. Any protocol is determined individually by the physician after a Good Faith Examination and current bloodwork. Some compounded medications used in physician-prescribed protocols are not FDA-approved. Results vary and are not guaranteed. Service is available only in California, where our prescribing physicians are licensed.

9. Price changes

We may change membership pricing with at least 30 days’ notice before your next renewal; continued membership after the effective date constitutes acceptance.

10. Governing law

This agreement is governed by the laws of the State of California.
