Telehealth Informed Consent

What Telehealth Is
Telehealth uses secure electronic communication — video, audio, or a combination — to allow a licensed clinician, and where applicable our supervising physician, to evaluate, diagnose, and consult with you when you are not in the same physical location. California law requires a Good Faith Exam — a documented clinical evaluation appropriate to the service requested — before we prescribe any medication, including peptide therapy.
Benefits and Risks
Telehealth allows you to receive care conveniently and can reduce delays in evaluation. As with any remote evaluation, there are limitations: the clinician cannot perform an in-person physical examination, technology can fail or be interrupted, and in rare cases information may be transmitted incorrectly due to technical issues. Your clinician will determine whether telehealth is appropriate for your situation, or whether an in-person visit is needed.
Confidentiality
Information disclosed during a telehealth visit is protected to the same standard as an in-person visit, as described in our HIPAA Notice of Privacy Practices.
Emergencies
Telehealth is not appropriate for medical emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room.
Your Right to Decline
Participation in telehealth is voluntary. You may withdraw consent at any time without affecting your right to future care, and you may request an in-person alternative where available.
Acknowledgment
By proceeding with a telehealth evaluation, you confirm that you have read and understood this consent, had the opportunity to ask questions, and agree to receive care by telehealth under the terms described above.
